Cagrilintide + Semaglutide

The dual-pathway weight stack: GLP-1 plus amylin, one weekly pin each.

Why this stack

This pairing copies the pharma playbook: the combined CagriSema asset exists because amylin and GLP-1 suppress appetite through separate pathways, and the combination out-performed either agent alone in phase 2. Semaglutide slows gastric emptying and works the incretin axis; cagrilintide works satiety through the amylin receptor. Both are once weekly, which keeps the schedule simple.

How it is run

  • Both once weekly subcutaneous, same day, in separate draws or as the combined vial.
  • Escalate each per its own titration; nausea is the limiting factor and slower is better.
  • This is a marathon protocol: 16 to 20+ weeks, ending in a taper rather than a hard stop.

What the components report

Reported upsides

  • Semaglutide: Significant weight loss in clinical trials
  • Semaglutide: Improved glycaemic control and HbA1c
  • Cagrilintide: Amylin analog for appetite regulation
  • Cagrilintide: Complements GLP-1 therapy for additive weight loss

Cautions

  • Semaglutide: Nausea, vomiting and diarrhoea (common on escalation)
  • Semaglutide: Reduced appetite (intended effect)
  • Cagrilintide: Nausea at higher doses
  • Cagrilintide: Injection site reactions
